Previous work by McAuley et al. Attention, Perception, & Psychophysics, 82, 3222-3233, (2020), Attention, Perception & Psychophysics, 83, 2229-2240, (2021) showed that disruption of the natural rhythm of target (attended) speech worsens speech recognition in the presence of competing background speech or noise (a target-rhythm effect), while disruption of background speech rhythm improves target recognition (a background-rhythm effect). While these results were interpreted as support for the role of rhythmic regularities in facilitating target-speech recognition amidst competing backgrounds (in line with a selective entrainment hypothesis), questions remain about the factors that contribute to the target-rhythm effect. Experiment 1 ruled out the possibility that the target-rhythm effect relies on a decrease in intelligibility of the rhythm-altered keywords. Sentences from the Coordinate Response Measure (CRM) paradigm were presented with a background of speech-shaped noise, and the rhythm of the initial portion of these target sentences (the target rhythmic context) was altered while critically leaving the target Color and Number keywords intact. Results showed a target-rhythm effect, evidenced by poorer keyword recognition when the target rhythmic context was altered, despite the absence of rhythmic manipulation of the keywords. Experiment 2 examined the influence of the relative onset asynchrony between target and background keywords. Results showed a significant target-rhythm effect that was independent of the effect of target-background keyword onset asynchrony. Experiment 3 provided additional support for the selective entrainment hypothesis by replicating the target-rhythm effect with a set of speech materials that were less rhythmically constrained than the CRM sentences.

Understanding continuous speech with competing background sounds is challenging, particularly for older adults. One stimulus property that may aid listeners understanding of to-be-attended (target) material is temporal regularity (rhythm). In the context of speech-in-noise understanding, McAuley and colleagues recently showed a target rhythm effect whereby recognition of target speech was better when natural speech rhythm of a target talker was intact than when it was temporally altered. The current study replicates the target rhythm effect using a synthetic vowel sequence paradigm in young adults (Experiment 1) and then uses this paradigm to investigate potential age-related changes in the effect of rhythm on recognition (Experiment 2). Listeners identified the last three vowels of temporally regular (isochronous) and irregular (anisochronous) synthetic vowel sequences in quiet and with a competing background sequence of vowel-like harmonic tone complexes presented at various tempos. The results replicated the target rhythm effect whereby temporal regularity in the vowel sequences improved identification accuracy of young listeners compared to irregular vowel sequences. The magnitude of the effect was not found to be influenced by background tempo, but faster background tempos led to greater vowel identification accuracy independent of regularity. Older listeners also demonstrated a target rhythm effect but received less benefit from the temporal regularity of the target sequences than did young listeners. This study highlights the importance of rhythm for understanding age-related differences in selective listening in complex environments and provides a novel paradigm for investigating effects of rhythm on perception.

Sensitivity to the temporal properties of auditory patterns tends to be poorer in older listeners, and this has been hypothesized to be one factor contributing to their poorer speech understanding. This study examined sensitivity to speech rhythms in young and older normal-hearing subjects, using a task designed to measure the effect of speech rhythmic context on the detection of changes in the timing of word onsets in spoken sentences. A temporal-shift detection paradigm was used in which listeners were presented with an intact sentence followed by two versions of the sentence in which a portion of speech was replaced with a silent gap: one with correct gap timing (the same duration as the missing speech) and one with altered gap timing (shorter or longer than the duration of the missing speech), resulting in an early or late resumption of the sentence after the gap. The sentences were presented with either an intact rhythm or an altered rhythm preceding the silent gap. Listeners judged which sentence had the altered gap timing, and thresholds for the detection of deviations from the correct timing were calculated separately for shortened and lengthened gaps. Both young and older listeners demonstrated lower thresholds in the intact rhythm condition than in the altered rhythm conditions. However, shortened gaps led to lower thresholds than lengthened gaps for the young listeners, while older listeners were not sensitive to the direction of the change in timing. These results show that both young and older listeners rely on speech rhythms to generate temporal expectancies for upcoming speech events. However, the absence of lower thresholds for shortened gaps among the older listeners indicates a change in speech-timing expectancies with age. A further examination of individual differences within the older group revealed that those with better rhythm-discrimination abilities (from a separate study) tended to show the same heightened sensitivity to early events observed with the young listeners.

This study was designed to examine age effects on various auditory perceptual skills using a large group of listeners (155 adults, 121 aged 60-88 years and 34 aged 18-30 years), while controlling for the factors of hearing loss and working memory (WM). All subjects completed 3 measures of WM, 7 psychoacoustic tasks (24 conditions) and a hearing assessment. Psychophysical measures were selected to tap phenomena thought to be mediated by higher-level auditory function and included modulation detection, modulation detection interference, informational masking (IM), masking level difference (MLD), anisochrony detection, harmonic mistuning, and stream segregation. Principal-components analysis (PCA) was applied to each psychoacoustic test. For 6 of the 7 tasks, a single component represented performance across the multiple stimulus conditions well, whereas the modulation-detection interference (MDI) task required two components to do so. The effect of age was analyzed using a general linear model applied to each psychoacoustic component. Once hearing loss and WM were accounted for as covariates in the analyses, estimated marginal mean thresholds were lower for older adults on tasks based on temporal processing. When evaluated separately, hearing loss led to poorer performance on roughly 1/2 the tasks and declines in WM accounted for poorer performance on 6 of the 8 psychoacoustic components. These results make clear the need to interpret age-group differences in performance on psychoacoustic tasks in light of cognitive declines commonly associated with aging, and point to hearing loss and cognitive declines as negatively influencing auditory perceptual skills.

Recent work by McAuley et al. (Attention, Perception, & Psychophysics, 82, 3222-3233, 2020) using the Coordinate Response Measure (CRM) paradigm with a multitalker background revealed that altering the natural rhythm of target speech amidst background speech worsens target recognition (a target-rhythm effect), while altering background speech rhythm improves target recognition (a background-rhythm effect). Here, we used a single-talker background to examine the role of specific properties of target and background sound patterns on selective listening without the complexity of multiple background stimuli. Experiment 1 manipulated the sex of the background talker, presented with a male target talker, to assess target and background-rhythm effects with and without a strong pitch cue to aid perceptual segregation. Experiment 2 used a vocoded single-talker background to examine target and background-rhythm effects with envelope-based speech rhythms preserved, but without semantic content or temporal fine structure. While a target-rhythm effect was present with all backgrounds, the background-rhythm effect was only observed for the same-sex background condition. Results provide additional support for a selective entrainment hypothesis, while also showing that the background-rhythm effect is not driven by envelope-based speech rhythm alone, and may be reduced or eliminated when pitch or other acoustic differences provide a strong basis for selective listening.

The Test of Basic Auditory Capabilities (TBAC) is a battery of auditory-discrimination tasks and speech-identification tasks that has been normed on several hundred young normal-hearing adults. Previous research with the TBAC suggested that cognitive function may impact the performance of older adults. Here, we examined differences in performance on several TBAC tasks between a group of 34 young adults with a mean age of 22.5 years (SD = 3.1 years) and a group of 115 older adults with a mean age of 69.2 years (SD = 6.2 years) recruited from the local community. Performance of the young adults was consistent with prior norms for this age group. Not surprisingly, the two groups differed significantly in hearing loss and working memory with the older adults having more hearing loss and poorer working memory than the young adults. The two age groups also differed significantly in performance on six of the nine measures extracted from the TBAC (eight test scores and one average test score) with the older adults consistently performing worse than the young adults. However, when these age-group comparisons were repeated with working memory and hearing loss as covariates, the groups differed in performance on only one of the nine auditory measures from the TBAC. For eight of the nine TBAC measures, working memory was a significant covariate and hearing loss never emerged as a significant factor. Thus, the age-group deficits observed initially on the TBAC most often appeared to be mediated by age-related differences in working memory rather than deficits in auditory processing. The results of these analyses of age-group differences were supported further by linear-regression analyses with each of the 9 TBAC scores serving as the dependent measure and age, hearing loss, and working memory as the predictors. Regression analyses were conducted for the full set of 149 adults and for just the 115 older adults. Working memory again emerged as the predominant factor impacting TBAC performance. It is concluded that working memory should be considered when comparing the performance of young and older adults on auditory tasks, including the TBAC.

Three experiments investigated listeners' ability to use speech rhythm to attend selectively to a single target talker presented in multi-talker babble (Experiments 1 and 2) and in speech-shaped noise (Experiment 3). Participants listened to spoken sentences of the form "Ready [Call sign] go to [Color] [Number] now" and reported the Color and Number spoken by a target talker (cued by the Call sign "Baron"). Experiment 1 altered the natural rhythm of the target talker and background talkers for two-talker and six-talker backgrounds. Experiment 2 considered parametric rhythm alterations over a wider range, altering the rhythm of either the target or the background talkers. Experiments 1 and 2 revealed that altering the rhythm of the target talker, while keeping the rhythm of the background intact, reduced listeners' ability to report the Color and Number spoken by the target talker. Conversely, altering the rhythm of the background talkers, while keeping the target rhythm intact, improved listeners ability to report the Color and Number spoken by the target talker. Experiment 3, which embedded the target talker in speech-shaped noise rather than multi-talker babble, similarly reduced recognition of the target sentence with increased alteration of the target rhythm. This pattern of results favors a dynamic-attending theory-based selective-entrainment hypothesis over a disparity-based segregation hypothesis and an increased salience hypothesis.

BACKGROUND: Musicians are known to be at risk for developing hearing sensitivity and hearing-related problems given their occupational exposure to high-level sound. Among options for hearing conservation, earplugs are an effective and inexpensive choice. Adoption rates for musicians' earplugs remains consistently low, however, given concerns about the impact of hearing protection on their own performance as well as concerns that the resultant music will be a negative experience for listeners. In fact, few studies have (1) examined musicians' attitudes about using hearing protection while performing themselves and (2) determined whether music played by musicians wearing hearing protection sounds different to listeners. PURPOSE: The purposes of this study were (1) to evaluate how wearing musicians' earplugs affected musicians' perception of their performance while they were playing, and (2) to examine whether listeners can distinguish a difference between music recorded by musicians playing with and without earplugs. RESEARCH DESIGN: Experiment 1: student musicians were recorded playing under two conditions (with and without wearing earplugs) and then were surveyed about their experience. Experiment 2: musically experienced and naïve listeners were presented with musical samples played by musicians with and without earplugs in an ABX format. Listeners responded by indicating whether the third stimulus (X) was conditionally identical to the first (A) or second stimulus (B). RESULTS: Experiment 1: while performing, musicians always preferred the no earplugs condition. The majority, however, rated the overall experience of playing with earplugs as generally positive. Experiment 2: listeners were unable to hear a difference between the two recordings. DISCUSSION: In this experiment, musicians rated their experience playing without hearing protection more favorably than their experience playing with hearing protection, but most musicians rated their experience with hearing protection as generally positive. The inability of listeners to distinguish a difference in music played with and without hearing protection suggests that the listening experience may not be adversely impacted by hearing protection worn by the performers. CONCLUSION: Earplugs are an inexpensive, noninvasive strategy for hearing conservation for musicians, and this study indicates that barriers to wearing hearing protection might be less problematic than previously reported.

Purpose: Three experiments examined the use of competing coordinate response measure (CRM) sentences as a multitalker babble. Method: In Experiment I, young adults with normal hearing listened to a CRM target sentence in the presence of 2, 4, or 6 competing CRM sentences with synchronous or asynchronous onsets. In Experiment II, the condition with 6 competing sentences was explored further. Three stimulus conditions (6 talkers saying same sentence, 1 talker producing 6 different sentences, and 6 talkers each saying a different sentence) were evaluated with different methods of presentation. Experiment III examined the performance of older adults with hearing impairment in a subset of conditions from Experiment II. Results: In Experiment I, performance declined with increasing numbers of talkers and improved with asynchronous sentence onsets. Experiment II identified conditions under which an increase in the number of talkers led to better performance. In Experiment III, the relative effects of the number of talkers, messages, and onset asynchrony were the same for young and older listeners. Conclusions: Multitalker babble composed of CRM sentences has masking properties similar to other types of multitalker babble. However, when the number of different talkers and messages are varied independently, performance is best with more talkers and fewer messages.

BACKGROUND: The sooner people receive treatment for hearing loss (HL), the quicker they are able to recognize speech and to master hearing aid technology. Unfortunately, a majority of people with HL wait until their impairments have progressed from moderate to severe levels before seeking auditory rehabilitation. To increase the number of individuals with HL who pursue and receive auditory rehabilitation, it is necessary to improve methods for identifying and informing these people via widely accessible hearing screening procedures. Screening for HL is the first in a chain of events that must take place to increase the number of patients who enter the hearing health-care system. New methods for hearing screening should be readily accessible through a common medium (e.g., telephone or computer) and should be relatively easy and quick for people to self-administer. PURPOSE: The purpose of this study was to assess a digits-in-noise (DIN) hearing screening test that was delivered via personal computer. RESEARCH DESIGN: Participants completed the Hearing Handicap Inventory for Adults (HHIA) questionnaire, audiometric testing in a sound booth, and computerized DIN testing. During the DIN test, sequences of three spoken digits were presented in noise via headphones at varying signal-to-noise ratios (SNRs). Participants entered each three-digit sequence they heard using an on-screen keypad. STUDY SAMPLE: Forty adults (16 females, 24 males) participated in the study, of whom 20 had normal hearing and 20 had HL (pure-tone average [PTA] thresholds for 0.5, 1, 2, and 4 kHz >25 dB HL). DATA COLLECTION AND ANALYSIS: DIN SNR and PTA data were analyzed and compared for each ear tested. Receiver operating characteristic curves based on these data were plotted. A measure of overall accuracy of a screening test is the area under the receiver operating characteristic curve (AUC). This measures the average true positive rate across false positives at varying DIN SNR cutoffs. Larger values of the AUC indicate, on average, more accurate screening tests. HHIA responses were analyzed and compared to PTA and DIN SNR results using Pearson correlation statistics. RESULTS: HHIA scores were positively correlated with audiometric PTA and DIN SNR results (p < 0.001 for all correlations). For an HL criterion of one or more frequencies from 0.25 to 8 kHz >25 dB HL, the AUC for the DIN test was 0.95. When a criterion of hearling level was set at one or more frequencies from 0.25 to 8 kHz >20 dB HL, the AUC for the DIN test was 0.96. CONCLUSIONS: The computer version of the DIN test demonstrated excellent sensitivity and specificity for our sample of 40 participants. AUC results (≥0.95) suggest that this DIN test administered via computer should be very useful for adult hearing screening.

The Speech Intelligibility Index (SII) assumes additivity of the importance of acoustically independent bands of speech. To further evaluate this assumption, open-set speech recognition was measured for words and sentences, in quiet and in noise, when the speech stimuli were presented to the listener in selected frequency bands. The filter passbands were constructed from various combinations of 20 bands having equivalent (0.05) importance in the SII framework. This permitted the construction of a variety of equal-SII band patterns that were then evaluated by nine different groups of young adults with normal hearing. For monosyllabic words, a similar dependence on band pattern was observed for SII values of 0.4, 0.5, and 0.6 in both quiet and noise conditions. Specifically, band patterns concentrated toward the lower and upper frequency range tended to yield significantly lower scores than those more evenly sampling a broader frequency range. For all stimuli and test conditions, equal SII values did not yield equal performance. Because the spectral distortions of speech evaluated here may not commonly occur in everyday listening conditions, this finding does not necessarily represent a serious deficit for the application of the SII. These findings, however, challenge the band-independence assumption of the theory underlying the SII.

OBJECTIVE: The Dutch digits-in-noise test (NL DIN) and the American-English version (US DIN) are speech-in-noise tests for diagnostic and clinical usage. The present study investigated differences between NL DIN and US DIN speech reception thresholds (SRTs) for a group of native Dutch-speaking listeners. DESIGN: In experiment 1, a repeated-measures design was used to compare SRTs for the NL DIN and US DIN in steady-state noise and interrupted noise for monaural, diotic, and dichotic listening conditions. In experiment 2, a subset of these conditions with additional speech material (i.e. US DIN triplets without inter-digit coarticulation/prosody) was used. STUDY SAMPLE: Experiment 1 was conducted with 16 normal-hearing Dutch students. Experiment 2 was conducted with nine different students. RESULTS: No significant differences between SRTs measured with the NL DIN and US DIN were found in steady-state noise. In interrupted noise the US DIN SRTs were significantly better in monaural and diotic listening conditions. Experiment 2 demonstrated that these better SRTs cannot be explained by the combined effect of inter-digit coarticulation and prosody in the American-English triplets. CONCLUSIONS: The NL DIN and US DIN are highly comparable and valuable tests for measuring auditory speech recognition abilities. These tests promote across-language comparisons of results.

A modified auditory n-back task was used to examine the ability of young and older listeners to remember the content of spoken messages presented from different locations. The messages were sentences from the Coordinative Response Measure (CRM) corpus, and the task was to judge whether a target word on the current trial was the same as in the most recent presentation from the same location (left, center, or right). The number of trials between comparison items (the number back) was varied while keeping the number of items to be held in memory (the number of locations) constant. Three levels of stimulus uncertainty were evaluated. Low- and high-uncertainty conditions were created by holding the talker (voice) and nontarget words constant, or varying them unpredictably across trials. In a medium-uncertainty condition, each location was associated with a specific talker, thus increasing predictability and ecological validity. Older listeners performed slightly worse than younger listeners, but there was no significant difference in response times (RT) for the two groups. An effect of the number back (n) was seen for both PC and RT; PC decreased steadily with n, while RT was fairly constant after a significant increase from n = 1 to n = 2. Apart from the lower PC for the older group, there was no effect involving age for either PC or RT. There was an effect of target word location (faster RTs with a late-occurring target) and an effect of uncertainty (faster RTs with a constant talker-location mapping, relative to the high-uncertainty condition). A similar pattern of performance was observed with a group of elderly hearing-impaired listeners (with and without shaping to ensure audibility), but RTs were substantially slower and the effect of uncertainty was absent. Apart from the observed overall slowing of RTs, these results provide little evidence for an effect of age-related changes in cognitive abilities on this task.

BACKGROUND: Several European countries have demonstrated successful use of telephone screening tests for auditory function. The screening test consists of spoken three-digit sequences presented in a noise background. The speech-to-noise ratios of the stimuli are determined by an adaptive tracking method that converges on the level required to achieve 50% correct recognition. PURPOSE: A version of the three-digit telephone screening protocol for the United States was developed: the US National Hearing Test (NHT). The objective of the current study was to determine the sensitivity and specificity as well as the feasibility of the NHT for use within the Department of Veterans Affairs (VA). Research Design and Study Sample: Using a multisite study design with convenience sampling, we used the NHT to collect data from 693 participants (1379 ears) from three geographical areas of the United States (Florida, Tennessee, and California). DATA COLLECTION AND ANALYSIS: The NHT procedures were as follows: the participants (1) called a toll-free telephone number, (2) entered their assigned ear-specific identification code, (3) listened to 40-sets of digit triplets presented in speech-spectrum background noise, and (4) entered in the numbers that they heard on the telephone key pad. The NHT was performed on each ear, either at home or in a VA clinic. In addition to collecting data from the experimental task, we gathered demographic data and the data from other standard-of-care tests (i.e., audiometric thresholds and speech recognition tests in quiet and in noise). RESULTS: A total of 505 participants completed the NHT at a VA clinic, whereas 188 completed the test at home. Although the ear-specific NHT and mean pure-tone threshold all correlated significantly (p < 0.001), there were more modest correlations in the low- and high-frequency ranges with the highest correlation seen with the 2000 Hz mean pure-tone threshold. When the NHT 50% point or threshold was compared with the three-frequency PTA at 500, 1000, and 2000 Hz, the sensitivity was 0.87 and specificity was 0.54. When comparing the NHT with the four-frequency PTA at 500, 1000, 2000, and 4000 Hz, the sensitivity was 0.81 and specificity increased to 0.65. The NHT also correlated strongly with other speech-in-noise measures. CONCLUSIONS: The NHT was found to correlate with other audiometric measures, including pure-tone thresholds and speech recognition tests in noise, at sufficiently high correlation values to support its use as a screening test of auditory function.

This study was designed to address individual differences in aided speech understanding among a relatively large group of older adults. The group of older adults consisted of 98 adults (50 female and 48 male) ranging in age from 60 to 86 (mean = 69.2). Hearing loss was typical for this age group and about 90% had not worn hearing aids. All subjects completed a battery of tests, including cognitive (6 measures), psychophysical (17 measures), and speech-understanding (9 measures), as well as the Speech, Spatial, and Qualities of Hearing (SSQ) self-report scale. Most of the speech-understanding measures made use of competing speech and the non-speech psychophysical measures were designed to tap phenomena thought to be relevant for the perception of speech in competing speech (e.g., stream segregation, modulation-detection interference). All measures of speech understanding were administered with spectral shaping applied to the speech stimuli to fully restore audibility through at least 4000 Hz. The measures used were demonstrated to be reliable in older adults and, when compared to a reference group of 28 young normal-hearing adults, age-group differences were observed on many of the measures. Principal-components factor analysis was applied successfully to reduce the number of independent and dependent (speech understanding) measures for a multiple-regression analysis. Doing so yielded one global cognitive-processing factor and five non-speech psychoacoustic factors (hearing loss, dichotic signal detection, multi-burst masking, stream segregation, and modulation detection) as potential predictors. To this set of six potential predictor variables were added subject age, Environmental Sound Identification (ESI), and performance on the text-recognition-threshold (TRT) task (a visual analog of interrupted speech recognition). These variables were used to successfully predict one global aided speech-understanding factor, accounting for about 60% of the variance.

OBJECTIVE: In this study, the effects of age, hearing loss, and modality on the ability to integrate partial information in degraded stimuli, either speech or text, were examined using isolated words. It was hypothesized that the ability to make use of partial information in speech diminishes with age. It was also hypothesized that additional contributions of cochlear pathology underlying hearing loss would be manifest as a further decrement in performance for older adults with hearing loss, relative to older adults with normal hearing. Furthermore, it was hypothesized that, if the ability to integrate partial information in speech is amodal, then recognition performance for degraded speech would be associated with recognition performance for parallel measures of degraded text. Last, it was hypothesized that, if the nature of the amodal ability to integrate partial information is cognitive, then the performance on auditory and visual measures of word recognition would be correlated with performance on measures of working memory. DESIGN: Twenty-five young adults with normal hearing, 20 older adults with normal hearing, and 21 older adults with hearing loss participated in this study. All participants completed three auditory and two parallel visual tasks consisting of listening to or reading degraded words or text. Older participants also completed a working-memory test battery. Group effects were examined for each of the auditory and visual measures. Performance of older participants on cognitive measures was compared with available data from a younger group participating in a different study in our laboratory (with similar protocol). Correlations between auditory and visual measures of speech recognition were examined for all participants. In addition, correlations between perceptual and cognitive measures were computed for the older participants. Finally, the relationship between dependent auditory measures and other independent measures in older adults were further examined using stepwise linear regression analyses. RESULTS: Of the 10 possible comparisons between the young and the two older groups for the five primary dependent measures, the young performed significantly better than the elderly did, 8 of the 10 times. The two older groups performed similarly for most tasks. In young adults, performance among the auditory tasks and between the two visual tasks was significantly and moderately to strongly correlated. In addition, performance on one of the visual tasks was weakly to moderately significantly correlated with performance on each of the three auditory tasks. Similar moderate to strong correlations were found within the auditory and visual modalities in older adults. However, none of the between-modality correlations were significant in the elderly. CONCLUSIONS: In summary, the results of this study suggest that the ability to integrate partial information in degraded words diminishes with age. Once audibility is accounted for, this ability does not seem to diminish with cochlear pathology. In young adults, both modality-specific factors and amodal cognitive factors seem to contribute to this ability. In older adults, although modality-specific factors continue to be important, it seems that the perceptual mechanisms that underlie the processing of degraded speech and text are separate, at least for isolated words. Our results suggest that, when peripheral factors are accounted for, some higher-level, yet-to-be identified, age-related factors contribute to speech-communication difficulties in the elderly.

BACKGROUND: An estimated 36 million US citizens have impaired hearing, but nearly half of them have never had a hearing test. As noted by a recent National Institutes of Health/National Institute on Deafness and Other Communication Disorders (NIH/NIDCD) Working Group, "In the United States (in contrast to many other nations) there are no readily accessible low cost hearing screening programs…" (Donahue et al, 2010, p. 2). Since 2004, telephone administered screening tests utilizing three-digit sequences presented in noise have been developed, validated, and implemented in seven countries. Each of these tests has been based on a test protocol conceived by Smits and colleagues in The Netherlands. PURPOSE: Investigators from Communication Disorders Technology, Inc., Indiana University, and VU University Medical Center of Amsterdam agreed to collaborate in the development and validation of a screening test for hearing impairment suitable for delivery over the telephone, for use in the United States. This test, utilizing spoken three-digit sequences (triplets), was to be based on the design of Smits and his colleagues. RESEARCH DESIGN: A version of the digits-in-noise test was developed utilizing digit triplets spoken in Middle American dialect. The stimuli were individually adjusted to speech-to-noise ratio (SNR) values yielding 50% correct identification, on the basis of data collected from a group of 10 young adult listeners with normal hearing. A final set of 64 homogeneous stimuli were selected from an original 160 recorded triplets. Each test consisted of a series of 40 triplets drawn at random, presented in a noise background. The SNR threshold for 50% correct identification of the triplets was determined by a one-down, one-up adaptive procedure. The test was implemented by telephone, and administered to listeners with varying levels of hearing impairment. The listeners were then evaluated with pure-tone tests and other audiometric measures as clinically appropriate. STUDY SAMPLE: Ninety participants included 72 who were volunteers from the regular client population at the Indiana University Hearing Clinic, and 18 who were recruited with a newspaper ad offering a free hearing test. Of the 90 participants, 49 were later determined to have mean pure-tone thresholds greater than 20 dB hearing level (HL). DATA COLLECTION AND ANALYSIS: The primary data analyses were correlations between telephone test thresholds and other measures, including pure-tone thresholds and speech recognition tests, collected for the same participants. RESULTS: The correlation between the telephone test and pure-tone thresholds (r = 0.74) was within the range of correlations observed with successful telephone screening tests in use in other countries. Thresholds based on the average of only 21 trials (trials five through 25 of the 40-trial tracking history) yielded sensitivity and specificity values of 0.80 and 0.83, respectively, using pure-tone average((0.5, 1.0, 2.0 kHz)) >20 dB HL as the criterion measure. CONCLUSIONS: This US version of the digits-in-noise telephone screening test is sufficiently valid to be implemented for use by the general public. Its properties are quite similar to those telephone screening tests currently in use in most European countries. Telephone tests provide efficient, easy to use, and valid screening for functional hearing impairment. The results of this test are a reasonable basis for advising those who fail to seek a comprehensive hearing evaluation by an audiologist.

The ability to recognize spoken words interrupted by silence was investigated with young normal-hearing listeners and older listeners with and without hearing impairment. Target words from the revised SPIN test by Bilger et al. [J. Speech Hear. Res. 27(1), 32-48 (1984)] were presented in isolation and in the original sentence context using a range of interruption patterns in which portions of speech were replaced with silence. The number of auditory "glimpses" of speech and the glimpse proportion (total duration glimpsed/word duration) were varied using a subset of the SPIN target words that ranged in duration from 300 to 600 ms. The words were presented in isolation, in the context of low-predictability (LP) sentences, and in high-predictability (HP) sentences. The glimpse proportion was found to have a strong influence on word recognition, with relatively little influence of the number of glimpses, glimpse duration, or glimpse rate. Although older listeners tended to recognize fewer interrupted words, there was considerable overlap in recognition scores across listener groups in all conditions, and all groups were affected by interruption parameters and context in much the same way.

Four experiments investigated the acoustical correlates of similarity and categorization judgments of environmental sounds. In Experiment 1, similarity ratings were obtained from pairwise comparisons of recordings of 50 environmental sounds. A three-dimensional multidimensional scaling (MDS) solution showed three distinct clusterings of the sounds, which included harmonic sounds, discrete impact sounds, and continuous sounds. Furthermore, sounds from similar sources tended to be in close proximity to each other in the MDS space. The orderings of the sounds on the individual dimensions of the solution were well predicted by linear combinations of acoustic variables, such as harmonicity, amount of silence, and modulation depth. The orderings of sounds also correlated significantly with MDS solutions for similarity ratings of imagined sounds and for imagined sources of sounds, obtained in Experiments 2 and 3--as was the case for free categorization of the 50 sounds (Experiment 4)--although the categorization data were less well predicted by acoustic features than were the similarity data.
